Fees and Billing Policy

Counseling Services fees are as followed:

Initial Intake
$135.00: Individual therapy session

Sessions are 45 to 50 minutes long. Everything
will be done to try and keep sessions at this length so that I can see all my clients on a timely manner. Thank
you for your cooperation and understanding.
*Ancillary services:
From time to time, your therapist may be required to provide services which are not reimbursable by insurance or other
third party payors. These services may possibly include report writing, testifying
in court, staffings, travel time outside the office to another site with regard to your case.
PAYMENT FOR SERVICES: You are responsible for
your charges. Every other month, statements will be sent to you to keep you informed about your account.
There are two payment options available: cash or insurance. Each of these has
certain advantages which may suit your needs.
Cash Accounts:
Advantages- Protects your
privacy by avoiding insurance industry access to information concerning your treatment; and it frees your therapist to pursue
a course of therapy without managed care restrictions and delays due to policies of your health plan.
Policies-Clients who choose to use the
cash option and pay for that in full each date of service may be eligible for a cash discount.
Insurance Accounts:
In most cases, Clearwater Counseling Services will be able to bill your insurance company directly.
However, this is a service I provide for you and it carries no guarantee of third party coverage. Your
insurance plan is a contract between you and your carrier and you always remain responsible for your bill. Policies-Most insurance plans do not cover 100% therefore,
a co-payment is due from you on the Date of Service (DOS). If you elect to have us bill your insurance
company, in most circumstances, you will have 90 days from the DOS to pay the balance in full, regardless of whether or not
your insurance company has responded (most insurance companies reimburse within 60 days of billing.) Deductibles
that are not met require payment in full on the DOS. Insurance requires a medical diagnosis for each procedure.
Your plan may exclude certain diagnoses. If so, you will be responsible for charges.
Note: Most psychological testing, marriage therapy, and family therapy is not insurance reimbursable.
Check with your psychologist for clarification. Some insurance plans or managed care contracts require treatment plans or contact for your therapist to pre-certify
further treatment. You will be asked to sign a form granting your therapist permission to make this contact
in order to use your insurance benefit. Our
strongest recommendation is that if you choose to utilize insurance to pay for therapy, stay well-informed regarding your
policy. I will do what I can to assist you with this, but it is ultimately your responsibility. Note: If you are unable to
complete your therapy due to financial hardship and if you are willing to discuss these concerns with myself it may be possible
to work out a payment plan that is more suitable to your financial situation without jeopardizing the financial operation
of this clinic.
LATE CANCELLATIONS AND NO-SHOWS:
24 hour notice must
be given or a $80.00 fee will be charged. Failed appointments may jeopardize continued service at Clearwater
Counseling Service. I am unable to charge no-show or late cancellation fees to clients who have medical
assistance; therefore two or more late cancellations and/or no shows will result in termination of services at Clearwater
Counseling Services.
DELINQUENT ACCOUNTS: A client can maintain no more than a 200.00 balance of personal responsibility
without jeopardizing use of the clinic services. I reserve the right to terminate services with a client
who is failing to maintain his/her financial responsibility.
COLLECTIONS: When
it becomes necessary to use a collection agency to resolve a past due account, and additional 30% will be assessed to your
account to cover the costs of this action.
